    Description

    At AWARE, we truly value the importance of families and understand that our connections with communities are crucial. We are currently looking for caring and dedicated individuals to join our team as an SMDI Case Manager.

    Severe and Disabling Mental Illness (SDMI) Waiver Case Management is a program provided by AWARE for individuals aged 18 and above who need ongoing community support for mental health issues.

    Responsibilities:

    • Offer case management services to adults with severe mental illnesses as designated by the State of Montana.
    • Assist individuals in accessing medical, social, educational, vocational, and recreational services.

    Requirements:

    • Excited to be part of our team? AWARE's SDMI Case Managers receive $22.19 per hour.
    • Talents, skills, and abilities:
      • Develop and execute individual treatment plans based on AWARE's Unconditional Care Philosophy.
      • Possess strong interpersonal communication skills for teamwork, observation, and reporting.
      • Hold a BS/BA Degree in a Human Services Field and have at least one year of direct work experience.
