- Assess clients' needs, including their activities of daily living, every 90 days.
- Coordinate referrals and provide access to resources and service providers to address clients' needs effectively.
- Engage with clients and relevant individuals in treatment planning every 90 days.
- Represent Winds of Change in a professional and respectful manner at all times.
- Record daily progress notes accurately and comprehensively.
- Complete internal and external documentation promptly and accurately.
- Participate in mandatory training sessions, whether online or in person.
- Hold a valid driver's license and maintain transportation with automobile insurance coverage.
- Strictly adhere to HIPAA regulations and uphold client confidentiality.
- Meet productivity standards consistently.
- Attend necessary meetings or events on a weekly basis.
- Maintain excellent attendance records.
- Familiarity with and adherence to HIPAA regulations.

In this role, you will serve as a committed advocate for individuals in our care, assisting them on their mental health journey and aiding them in accessing government benefits.
Key responsibilities entail effectively representing clients, participating in court proceedings, and offering valuable insights on specific diagnoses. Candidates must possess a bachelor's degree in social work, Psychology, or a related field of study with at least 1 year of work experience working with individuals facing severe mental illness (SDMI).
