- Conduct comprehensive assessments to identify clients' needs and develop personalized care plans
- Assist clients in accessing and utilizing community resources, including mental health services, housing programs, and government benefits.
- Advocate for clients' rights and needs within various systems, including healthcare, social services and legal services
- Provide crisis intervention and support as needed
- Collaborate with other healthcare professionals, social workers, and service providers to coordinate comprehensive care
- Monitor clients' progress and adjust care plans as necessary to ensure optimal outcomes
- Maintain accurate and up-to-date client records and documentation
- Educate clients and their families about mental health issues, available resources, and coping strategies
- Participate in team meetings, training sessions, and professional development opportunities
- Hold a valid driver's license and maintain transportation with automobile insurance coverage.
- Strictly adhere to HIPAA regulations and uphold client confidentiality.
- Meet productivity standards consistently.
- Maintain excellent attendance records.
- Familiarity with and adherence to HIPAA regulations.
- Ability to maintain a high level of confidentiality and discretion.

We are a Recovery Friendly workplace, looking for a dedicated and compassionate Mental Health Case Manager to join our team. In this role, you will serve as an advocate for individuals on their mental health journey, helping them access government assistance, secure housing for the homeless, support client recovery, and more Your efforts will make a significant impact on the lives of those facing severe mental illness.
