    Job Description

    SCOPE OF POSITION
    Assists in managing and coordinating organization-wide process improvement and clinical outcomes optimization efforts by utilizing a data-driven focus to prioritize initiatives. The Coordinator will ensure initiatives align with current evidence-based, best practice recommendations and will use clinical databases and national benchmarking to continuously identify opportunities for improved outcomes.
    • Accountable for facilitating progress toward desired transformational change and strategies for process improvement.
    • Utilizes process improvement methodology.
    • Develop strategies for improvement that includes specific outcomes and metrics to monitor progress to a goal and make recommendations for improvement.
    • Integrate technology tools to optimize process workflows.
    • Engages stakeholders across the system to ensure process alignment and prioritize resources; validates the merit and applicability of initiatives across functions related to clinical outcomes
    • Contribute and co-create with internal partners in the development of project management practices such as processes, tools, reporting and dashboard.
    • Demonstrates collaboration with leaders, associates, and physicians, fostering productive and supportive working relationships.
    • Uses data, such as National Surgical Quality Improvement Program (NSQIP) and AHRQ Patient Safety Indicator rates to prioritize perioperative initiatives for improvement.
    • Strong ability to lead process improvement teams in a collaborative manner with stakeholders across the organization aligned with the mission, vision, values, and strategic goals.
    • Collaborate with the staff to identify systems issues that impact the care delivery system
    • Participate in the evaluation, selection and integration of new and improved technology to improve clinical workflow and patient outcomes
    • Remain current with trends in clinical transformation and outcomes management and provides the staff with the resources and environment conducive to learning
    • Facilitate change and fosters a positive image of clinical outcomes management
    • Demonstrate excellent team building and collaboration skills with colleagues

      EDUCATION AND EXPERIENCE
      Graduate of an appropriate health program with 3-5 years of experience in healthcare quality and/or performance improvement. Bachelor degree required, masters preferred
      Licensure appropriate for respective area of healthcare experience.
      CPHQ Certification or willingness to pursue is preferred.
