- Actively participate in interdisciplinary staff meetings to collaborate with professionals from various fields, ensuring a holistic approach to client care and effective communication among team members.
- Thoroughly complete case management assessments on newly assigned clients after they have undergone a comprehensive diagnostic intake conducted by a licensed provider, ensuring all relevant information is gathered and documented.
- Provide a wide range of case management services, including conducting intake assessments, developing personalized treatment plans, setting achievable goals, monitoring progress regularly, and planning for client discharge, to support clients in their recovery journey.
- Regularly meet with clients and fellow staff members to review the clients' progress towards achieving their service goals. Make necessary modifications to the service plans to ensure they remain effective and relevant to the clients' evolving needs.
- Conduct daily therapy sessions and educational groups as assigned, utilizing evidence-based practices to facilitate client learning and therapeutic progress, fostering a supportive group environment.
- Meet with clients on a regular basis, either weekly or biweekly, to identify their needs, monitor their progress in treatment, and provide ongoing support and adjustments to their treatment plans as necessary.
- Consistently participate in regular weekly staff meetings, engage in staff training programs, and attend supervisory sessions to enhance professional development, stay updated on best practices, and maintain high standards of care.
- Maintain availability for alternate schedules, including evening and weekend shifts, to meet the diverse needs of clients and comply with the requirements of state and regulatory agencies, ensuring continuous and accessible client care.
- Bachelors Degree in related field, preferred
- Certification or Registration as a Chemical Dependency Counselor or related mental health field
- Minimum of 2 years of experience in the field of chemical dependency counseling.
