    Description

    JOB SUMMARY

    The Justice Enhanced Case Manager provides linkage and navigation services to who are involved in the justice system. The Justice Enhanced Case Manager coordinates care for clients and assists them in accessing services and resources that will help maintain them in the community at the lease restrictive level of care possible. The Justice Enhanced Case Manager acts as a liaison between treatment providers/community resources and clients. The Justice Enhanced Case Manager offers extensive knowledge of community resources and support as a primary means of assisting each client to increase their self-sufficiency and reduce recidivism rates.

  • Conducts intake interviews and develops case plans inclusive of all needs, including but not limited to, housing, employment, substance abuse or mental health treatment, and benefits.
  • Meets with clients while incarcerated as required by the program.
  • Coordinates proposed treatment placements with providers.
  • Gathers information regarding available services within the community and updates the team.
  • Maintains consistent contact with clients to remain informed on clients' progress and needs.
  • Provides on-call and after-hours services as required by contract and assigned.
  • Ability to problem solve, set priorities, and react appropriately to emergency situations
  • Ability to work independently and effectively collaborate with a team.
  • Ability to maintain the distinction between therapeutic and social relationships.
  • Ability to maintain positive morale and unit cohesion as evidenced by maintaining a cooperative and flexible attitude toward coworkers, showing adaptability to change, exhibiting effective communication and interpersonal skills, and taking initiative to solve problems.
  • Complies with all MHS and program rules, policies and procedures including HIPAA standards.
  • QUALIFICATIONS

  • For Master's level position: MA/MS degree in behavioral science or vocational rehabilitation fields.
  • For Bachelor's level position: BA/BS degree in social/human services field and 2 years' experience providing services to reentry, justice involved, or psychiatric populations.
  • For AA level position: AA in social services field and 3 years' experience providing services to reentry, justice involved, or psychiatric populations.
  • Knowledgeable of Cultural and socio-economic factors that inform service provision to persons with psychiatric or dual diagnosis disorders.
  • Knowledgeable of procedure manuals and has ability to interpret these effectively to staff, family members and community providers.
  • Fluency in English required; Fluency in Spanish preferred.
  • Ability to interact positively with persons of diverse socio-economic backgrounds and education.
  • Must be able to communicate clearly, both in writing and orally, with employees, management and co-workers
  • Proficient user of Microsoft Office Suite and EHR software
