    Job Overview

    The Linkage to Care Coordinator (LCC) facilitates client and community member referrals to AltaMed health services and focuses on retaining existing AltaMed patients.

    The LCC acts as a liaison between community members, clients and patients and AltaMed to support access to desired healthcare services.

    Responsibilities include scheduling appointments and addressing participant/patient inquiries, questions and concerns, including enrollment and eligibility and ensuring that their experience when accessing AltaMed Healthcare Services is of the highest quality.

    The LCC assists in health education and counseling services delivered through multiple programs and Women's Health strategic initiatives, including FamilyPACT, Comprehensive Perinatal Services (CPSP), teen pregnancy prevention and family planning programs.

    The LCC is also responsible for conducting outreach/marketing efforts to community partners.
    Minimum Requirements

    • High school diploma or equivalent required; Associates' Degree or courses toward Bachelor's Degree highly preferred.
    • A minimum of 1 year of women's health or OBGYN experience required.
    • A minimum of 2 years' experience in social service or health services related field required.
    • Knowledge of AltaMed health services, including but not limited to, FamilyPACT and CPSP.
    • Bilingual (English and Spanish) required.
