- Facilitating the care needs of Veterans needing inpatient, outpatient, home health in the community as well as traveling and permanently relocating Veterans seeking care at alternate VA medical facilities.
- Monitoring/following up, and reporting test results.
- Responsible and accountable for all elements of the nursing process when providing and/or supervising/coordinating patient care
- Assesses, plans, implements and evaluates care based on age-specific components
- Assumes responsibility for the coordination of care focused on patient transition through the continuum of care, patient and family education, patient self-management after discharge, and supporting factors that impact customer satisfaction
- Responsible for managing additional request for care, creating individualized plans of care, conducting follow up throughout the episode of care as needed; Educating on department policy & procedures
- Considers all characteristics of the individual, including age and life stages, state of health, race and culture, values, and previous experiences
- Influences care outcomes by collaborating with administrative and clinical staff within the department and throughout the facility
- Core elements of performance include knowledge and active participation in unit/program level quality improvement processes and initiatives as well as customer service programs
- Provide population specific and age-specific care for patients from young adulthood to older veterans/groups in the senior years of life.
- Manage a defined but dynamic caseload of patients in collaboration with the healthcare team.
- Initiate, plan, implement, evaluate, and appropriately document patient care based on identified measurable outcomes in the applicable assigned patient care environment.
- Ability to manage multiple/competing priorities
- Excellent communication and writing skills
- Anticipate and respond appropriately to emergencies, coordinate interventions, and evaluate responses through critical analysis of patient systems/operational situations.
- Initiate and lead individuals or groups through the educational process to optimize health. Monitor and document all applicable clinical reminders and performance measure outcomes timely and appropriately.
- Use Computerized Patient Record System (CPRS) in compliance with designated facility programs, ability to utilize computer systems.
- Computer Skills including but not limited to experience with Microsoft Word and Excel.
- Implement the National Patient Safety Goals to support positive outcomes for the patient group.

POSITION: Registered Nurse -Community Care Nurse CoordinatorThe Fayettteville NC VA Healthcare System is recruiting for three positions as Registered Nurse (Community Care Nurse Coordinator) in the Community Care Service. The Community Care Coordinators are critical to meet the projected workload demands for Community Care.
Duties
The Community Care Coordinator is aligned under the VA Community Care Program. The Community Care Coordinator will serve to assist Veterans and medical facility staff in scheduling appointments, transferring non-electronic records, arranging provider-to-provider contact. Other duties include, but not limited to;Pay: Competitive salary, regular salary increases, potential for performance awards
Paid Time Off: 50 days of paid time off per year (26 days of annual leave, 13 days of sick leave, 11 paid Federal holidays per year)
