    Position Title:Case Manager Date: 2/1/2022

    FLSA Status: Full Time, Non-Exempt Supervised by: Clinical Operations Director

    Primary Responsibilities: Under the supervision of the Clinical Operations Director, provide case management and support services to program participants.

    Essential Duties:

    • Be familiar with HUD HOPWA guidelines and program policies and procedures.
    • Help conduct intake assessments of applicants and assess eligibility criteria.
    • Facilitate new rental opportunities: determine household income, appropriate unit size, and fair market rent based on federal guidelines, issue search certificate; support client in housing search; perform inspections on units to meet the Housing Quality Standards (HQS); provide the Housing Assistance Payments contract to the landlord.
    • Explain Program and program participation to client: provide the Program Rules detailing various aspects of the program and their participation; develop agreements and housing case plan with clients. Strictly and fairly enforce the Program Rules and initiate disciplinary action when warranted.
    • Program participation: monitor client participation and compliance, conduct annual unit inspections and recertification, rent calculations, maintain client records,
    • Promote housing stability: conduct on-going assessments; amend housing case plans as needed; facilitate with and provide referral to supportive services.
    • Maintain up-to-date charting and documentation; ensure client confidentiality.
    • Telephone contact and/or home visit a minimum of twice a year. More frequent contacts depend on the needs of the client. Maintain written correspondences.
    • Maintain a list of landlord/housing resource and community supportive services.
    • Maintain a relationship with landlords to ensure that clients abide by lease agreement.
    • Maintain a client roster for annual reporting; assist with preparation of annual reports to the City & County and HUD.
    • Assist with the implementation of the Emergency Assistance and Temporary Shelter programs.

    Other Duties:

    • Assist the Executive Director, Clinical Operations Director, and other Program staff in their duties as required.
    • Represent Gregory House Programs in the community as requested and maintain good working relations with other agencies and persons providing services and support to people with HIV disease.

    Work Hours:

    Equipment Use: standard office equipment

    Physical, Mental, and Communication Demands: Requires use of discretion and judgment; requires working to established procedures, maintaining reliable work attendance; may require working under minimal supervision; requires verbal communication skills and following oral and written instructions.

    Skills/Knowledge: In addition to meeting the mental, physical, and communication demands listed above, requires knowledge of substance abuse and mentally ill issues and basic Microsoft Office applications.

    Education/Experience: At minimum, a Bachelor's degree and 2 years' social work experience in HIV, mental health, and/or substance use.
