    Description

    Job Summary

    Providing support and guidance to clients enrolled in a life management program, developing tailored program plans, and monitoring progress through mentoring sessions. Acting as an advocate to ensure clients have access to necessary services.

    Knowledge, Skills, and Abilities

    • Understanding of social service case management principles
    • Familiarity with community resources and agencies
    • Strong communication and motivation skills
    • Proficiency in Client Data Management Systems and outcome measurement
    • Ability to create and assess program plans based on client needs
    • Skilled at evaluating service levels and client progress
    • Capable of handling diverse client interactions
    • Experience in client interviews and data collection
    • Good organizational skills with attention to detail
    • Building professional relationships with clients and agencies

    Education and Experience

    Bachelor's degree in Social Work, Behavioral Science, or related field, with three years of direct case management experience. Alternatively, a combination of training and experience demonstrating required skills.

    Physical Requirements/Working Conditions

    • Meeting attendance requirements
    • Proficient in English language communication
    • Clear and effective oral and written communication
    • Ability to manage client caseload with clarity and efficiency
    • Capacity to handle multiple cases simultaneously
    • Physical tasks include light lifting and walking
    • Majority of work is seated with occasional standing or walking
    • Work environment includes exposure to health risks and driving for off-site visits
