- Facilitate access to community resources, including childcare services, educational support, health care, mental health services, and other services that contribute to family stability.
- Coordinate onsite events, groups, and workshops that focus on strengthening family relationships.
- Coordinate and collaborate with child welfare agencies, schools, and other service providers to ensure integrated and holistic support for families.
- Conduct regular home visits to assess the safety and stability of the housing environment and provide necessary support and interventions.
- Advocate for families within the program, ensuring their voices and perspectives are heard and considered in decision-making processes.
- Empower families to build their self-sufficiency and develop skills that support long-term housing stability and overall well-being.
- Maintain accurate and up-to-date participant records, case notes, and documentation in accordance with program guidelines and confidentiality standards.
- High School Diploma or equivalent
- 2 years experience in working with families
- Lived experience with homelessness or child welfare involvement preferred
- Clean driving record
