- Work with a Care Team of a Clinical Pharmacist and Care Coordinator to support patient and provider engagement for Care Transitions Management, Care Coordination, and Medication Therapy Management
- Engage high-risk patients telephonically to schedule appointments with Clinical Pharmacist for Comprehensive Medication Reviews
- Utilize multiple technology systems and outreach to provider offices and/or pharmacies as needed to capture pertinent information for patient engagement
- Support high-risk patients by scheduling PCP appointments on their behalf
- Prepare patient cases in workflow management platform for Clinical Pharmacist patient engagement
- Facilitate exchange of clinical information between Clinical Pharmacist and provider offices to ensure identified patient concerns and needs are addressed by provider offices
- Communicate with provider offices to problem-solve patient issues and/or facilitate communication between practice staff and Care Team
- Notify Clinical Pharmacist of relevant information gathered during calls to patient that may affect the patient's disease state or medication regimen
- Develop strong relationships with provider offices participating with Practice Extend program by communicating patient needs
- Build rapport and establish trusted relationships with patients via daily outbound and inbound calls
- High School Diploma / GED
- 2 years of Prior Medical / Healthcare experience in a hospital, physician's office, or pharmacy, providing administrative assistance or working with clinical documentation.
- 2 year experience working with medical terminology
- Computer proficiency, to include strong data entry and utilizing Microsoft Office, Microsoft Outlook, and Microsoft Teams
- Must be 18 years of age OR older
- Ability to work our normal business hours of 8:00am - 5:00pm.
- Pharmacy Tech, Pharmacy Tech for Insurance Company, or Medical Assistant
- Managed Care Experience
- Bilingual fluency in English and Spanish
- Reside within the state of Florida
- Ability to keep all company sensitive documents secure (if applicable)
- Required to have a dedicated work area established that is separated from other living areas and provides information privacy.
- Must live in a location that can receive a UnitedHealth Group approved high-speed internet connection or leverage an existing high-speed internet service.
- Able to take initiative and work independently
- Adaptable, flexible, and able to incorporate frequent process changes into established workflows within a fast-paced, dynamic environment

You will work alongside our Care Team's Clinical Pharmacist and Care Coordinator to improve the health outcomes of the patients we serve.
The Practice Extend
Clinical Administrative Coordinator is responsible for engaging high-risk patients for Comprehensive Medication Reviews with the Clinical Pharmacist, scheduling PCP appointments for patients on their behalf, assisting the Clinical Pharmacist with administrative tasks, and communicating with provider offices regarding patient medical history and current needs.
We offer 4 weeks of paid training. The hours during training will be 8:00am - 5:00pm.
