Description:
Completing data collection, closing gaps in care, and assisting in the coordination of care for patients. They will promote patient centered care and work collaboratively with primary care providers and the care teams.
Additional Skills & Qualifications:
1-3 years of healthcare background ideally from a back office healthcare role such as medical records/HIMs.
