- Provide remote medical consultations, diagnoses, and treatment plans for patients through telehealth platforms.
- Conduct comprehensive patient assessments, evaluate patient histories, and order and interpret diagnostic tests.
- Collaborate with physicians and other healthcare professionals to develop and implement patient care plans.
- Educate patients and their families about their health conditions, treatment options, and self-management strategies.
- Utilize remote patient monitoring technology to monitor and manage patients' health conditions.
- Document patient encounters accurately and in a timely manner in the electronic medical record system.
- Stay up-to-date with best practices in telehealth and remote patient monitoring and participate in ongoing education and training programs.
- Active and unrestricted license to practice as a Physician Assistant or Nurse Practitioner in the state of New York.
- Minimum of 3 years of experience in primary care, geriatrics, or a related field.
- Excellent communication skills and the ability to establish rapport with patients, families, and other healthcare professionals.
- Strong clinical skills and the ability to work independently.
- Experience with telehealth platforms and remote patient monitoring technology.
- Familiarity with electronic medical record systems and documentation requirements.
