    The Population Health Coordinator is an integral position that works with the clinic care team to proactively track the care needs of the patients on their panel and to ensure that patients receive the services they need to optimize their health and well-being. The diverse responsibilities can include, but are not limited to: encouraging patient engagement through direct contact with patients and their families, tracking and closing gaps in care, assuring accurate patient panels, coordination with health plans, and assimilation of multiple data sources to ensure an accurate picture of the quality of care of the patient population.

    Essential Functions
    • Lead the population management process by actively monitoring and sharing patient and population quality data with the care team in order to facilitate improvement in care outcomes.
    • Understand and champion proactive care for the entire population of patients connected with Metropolitan Pediatrics, not just those who come in for visits.
    • Maintain in-depth knowledge of all quality metrics for which the Organization is held accountable.
    • Participate as an active member of the clinic care team; provide diversified support to the team and to patients.
    • Proactively contact families/patients identified for health maintenance and other needed care.
    • Engage with families/patients through routine outreach utilizing phone contact, scheduled and unscheduled face-to-face meetings before, during, and after medical appointments.
    • Identify and decrease the impact of barriers interfering with a patient's access to health care.
    • Actively maintain the provider panel section of the clinic database.
    • Partner with clinical champions to ensure they have timely and accurate data to facilitate quality improvement efforts in various areas.
    • Ensure optimal monthly partial capitation and/or quality payment programs by maintaining accurate provider panel lists, timely onboarding of newly assigned patients, and working with health plans and families to move incorrectly assigned patients to their appropriate medical home.
    • Maintain confidentiality of sensitive patient information at all times.
    • Participate in site-specific and clinic-wide initiatives on an ongoing basis.
    • Attend and actively participate in huddles and team meetings.
    • Display high standards of office conduct.
    • Successfully work closely with others in a team setting.
    • Punctual, regular, timely, and dependable attendance.
    • Successfully work in a fast-paced, changing environment with effective time management and ability to multitask and prioritize workload.

    Qualifications

    Education/ Experience
    • High school education or equivalent.
    • 3+ years of experience in a clinical setting required.
    • Experience with panel management and quality metrics preferred.
    • Experience with EMR required, EPIC preferred.
    • Understanding of PCPCH (Patient-Centered Primary Care Home) requirements.
    • Knowledge and understanding of medical terminology and clinical procedures.
    • Minimum of 3 years of experience in customer service, computers, general office procedures, multitasking, and understanding of insurance terms.
