- Collaborates with the Performance Improvement Department and Clinical Operations in tracking agency performance.
- Responsible for gathering, compiling, analyzing data and producing information to promote effective Quality Care.
- Partner with an interdisciplinary teams within the agency.
- Advocate for and builds relationships with interdisciplinary team.
- Presents and reviews monthly performance scorecard.
- Participate in safety initiatives at the team and agency level to improve OASIS outcomes.
- Provide real time accountability and coaching related to OASIS documentation and Home Health Value Based Purchasing.
- Participates in Agency Quality Meetings
- Reports to Director of Performance Improvement

- Experience in Home Health Care
- Experience in Quality/Performance Improvement preferred
- Excellent communication and public speaking skills
- Skilled in the use of electronic medical record EPIC, MS Excel, MS Office, & MS PowerPoint
- Education: Graduate of an accredited School of Nursing required
- Bachelor of Science in Nursing (BSN) degree required with a Master of Science in Nursing (MSN) degree preferred
- Licensure: New York Registered Nurse (RN) License & Registration
