- Manage quality of the Supported Housing cluster documentation, accurate data, and conducting routine data maintenance
- Routinely run reports to identify gaps or delays with documentation
- Ensure the data collection is being collected accurate regarding mental health, health care, risk management, etc.
- Develop and monitor performance improvement plans to resolve data quality issues, including collaboration with ICL Hope, Compliance team, and program staff
- Review target goals are being meet by the teams and provide recommendations for improvements
- Create better data collection systems and establish policies to ensure documentation is up to standards with funders
- Ensure the data for Health Connect is being evaluated and reported to program staff
- Ensure the tracking of internal and external referrals
- Ensure housing vacancies report is accurately reported in collaboration with ICL Hope Team
- Work on other project assigned to support Supported Housing and Health Connect program
- Other assigned duties as necessary
- Ability to manage multiple projects and check on the completion of assigned tasks.
- Excellent Microsoft Suites(i.e., word document, excel, and power point)
- Strong interpersonal skills, emotional intelligence, and flexibility
- Proficient in computers and Electronic Health Record (EHR)
- Skilled at data management, evaluation, and improvement
- Ability to work independently and also collaboratively with other team members

JOB SUMMARY:
The Quality Improvement Specialist is responsible for data collection, data management, data reporting, and data outcomes. The Quality Improvement Specialist provides recommendation regarding strategies to support process and performance improvement. Also, will be responsible for ensuring all documentation reflects funding requirements and quality clinical care.
