    Position Summary

    We are looking for someone who is highly organized, works well in a team, and is a great communicator. As a residential service coordinator, you will be overlooking a home with up to four individuals, as well as the staff working in the home. You will be providing coordination for individuals with intellectual and developmental disabilities. As a part of their team, you will be ensuring the health and safety of the individuals. You will also be providing mentorship, guidance, and training to employees.

    Position Duties and Responsibilities

    1. Participate as an Interdisciplinary team member for all individuals on your caseload. Work with the individual and their team to identify current and future goals, assist in development, and ensure implementation of TSS and outcomes.
    2. Review written and verbal reports/ documentation related to daily progress and provide intervention or training to LSC's when needed.
    3. Ensure medical needs for the individual are met including appointments, medications, unplanned illness, or injury and all necessary follow-up for all areas of medical need to include regular communication with the assigned nurse/ nursing on-call.
    4. Advocate and teach others to advocate for the individuals supported.
    5. Act as a positive role model to both individuals and employees.
    6. Assist with hygiene, grooming, assist with medication delivery, housekeeping, laundry and cooking and any other areas identified within the ISP.
    7. Ensure the location presents a warm and inviting home setting.
    8. Provide emotional support and feedback to the individual and their family if applicable.
    9. Identify social, economic, recreational, and educational services in the community that will meet the individual's needs.
    10. Provide transportation (using personal vehicles) and ensure provisions for transport are available via LSC's on shift for the individuals served. Accessing the community is a priority and a necessity for those whom we support.
    11. Provide staffing coordination to meet state and agency required coverage to maintain health and safety. This includes covering the shifts when other coverage is unattainable.
    12. Provide Life Skills Coaches with strategies, tools and mentorship to ensure success in the following areas: the daily functions of providing a safe and inviting home experience, ISP implementation, increased independence of individuals supported, continuity of care, and promotion opportunities for employees.
    13. Evaluate employee's ability to successfully meet the needs of the individual. Provide formal and informal feedback / evaluations and training to the employee including 90-day assessment, annual evaluation, and infraction reports.
    14. Complete all required training including the DDSD service coordination requirements and work within all HeartWell Policies & Procedures.
    15. Maintain regular communication with the assigned nurse, SR service coordinator, program director and HR on all appropriate related matters.
    16. Active participation in all internal and external audits / surveys

    Listed below are direct tasking responsibilities. Please note this list is not all inclusive; additional tasks are necessary for the success of the home, the individuals and the employees providing the support.

    • On-call for home and agency in accordance with current guidelines
    • Utilize and manage agency petty cash card for location purchasing needs.
    • Manage day-to-day staffing needs.
    • Complete new hire training and support ongoing LSC / LSC II training and guidance
    • Conduct regular team meetings.
    • Weekly Medication QA's
    • documentation verifications including ensuring TSS frequency.
    • Time sheet review and submittal including working schedule.
    • Schedule and attend client appointments and ensure medication management.
    • Verify monthly location safety including safety drill completion.
    • Quarterly pharmacy verifications
    • Ensure nutritional recommendations are met through creating menus, prepping the shopping list and shopping for groceries or other items.
    • Teach Therap, GER process to LSC's and Support the ANE reporting process.
    • Client Funds reconciliation (ledger / receipts)
    • Attending meetings including administration, clinicals, ISPs
    • Provide oversite for secondary location as agency needs dictate (additional compensation of $400 bi-weekly will be provided while managing the secondary location)

    Requirements:

    Position Requirements

    • Minimum – High school diploma or equivalent
    • Current and Valid Driver's License
    • Safe insured/ registered vehicle
    • Must be 25 years of age
    • Completed a minimum of 1 yr. of human service management OR have successfully completed the HeartWell Management Training Program
