Description
This position is listed for the LAYC-MMYC Youth Drop-In Center at the Montgomery County Wheaton site.MoCo Reconnect serves youth ages 16-24 through education, workforce development, housing service support, drop-in services (hot showers, meals, laundry, recreation, etc.) and wrap-around case management support.
The lead case manager is responsible for providing housing leadership and guidance to all other case managers on the site and assisting the program manager with special projects.
The lead case manager will give participants one-on-one mentorship, case management services, and assistance and referrals to needed services.The lead case manager is also responsible for assigning clients to case managers and overseeing the input of data into HMIS (Montgomery County Housing database) and ETO (LAYC database) to ensure client assessments and profiles are completed and up to date.
The lead case manager is also responsible for ensuring the Individual Development Plan (IDP) for all clients is updated and completed.

ESSENTIAL RESPONSIBILITIES
Conduct assessments for new youth and develop and update the Individual Development Plans (IDP).
Demonstrate positive youth development skills and develop strong, healthy relationships with youth, family members, and others involved in the youth's life.
Monitor youth's progress and maintain frequent (at least biweekly) communication with youth through face-to-face meetings, phone calls, text messages, and any other means of communication to ensure strong connections and that individual goals are being addressed and met.
Make appropriate internal and external referrals, ensuring that required services are delivered on a timely basis.Work with community agencies, shelters, school leaders and staff, and all LAYC/MMYC program staff to develop and foster an effective coordination of services for youth.
Provide crisis intervention and coordinate response with the MoCo Reconnect Mental Health Counselor(s) when needed.Adhere to HIPPA regulations when preparing and maintaining case files for each participant, including enrollment and eligibility documents for housing and reengagement services.
Track youth outcomes, engagement, and participation in LAYC's Efforts-to Outcome (ETO) database system and Montgomery County's Homeless Management Information System (HMIS) database.
Submit monthly reports on caseload to Program Manager and keep roster spreadsheet updated.Work with the program manager to ensure programmatic benchmarks are met and consistent with DHHS policies and procedures.
Collect and maintain eligibility and enrollment documents, exit documents and notes.
Attend all prescribed meetings, trainings, and outreach events.
Provide day-to-day operational guidance to all case management staff across the department.
Establish a supportive network amongst all site Case Managers, identify training opportunities, and support the sharing resources and alignment of best practices across the department.
Serve as a resource to all staff on best practices for housing services and reengagement efforts for clients.Other assigned duties as needed.
EDUCATION & EXPERIENCE REQUIREMENTS
Bachelor's degree preferred; significant experience working with underserved youth will be considered.
Minimum 4 years of youth development experience.
Experience managing crisis situations.
Experience working with vulnerable populations preferred.
LAYC/MMYC requires the COVID-19 vaccine of all its employees. You will be asked to provide proof of vaccination at the time of your interview and at your onboarding.
Complete criminal and child abuse/neglect clearances.
SKILLS & QUALIFICATIONS
Strong interpersonal and organizational skills.
Knowledge of community and local housing resources.
Experience with HMIS and CRM data entry preferred.
Ability and desire to maintain a flexible schedule (school/community events, late nights, and weekends).
Experience of case management, monitoring and reporting.
Proficient computer skills, experience with ETO or other data collection systems is a plus.
Flexible, energetic, positive individual with commitment to youth development in a high quality, respectful environment.
Ability to work well and communicate with culturally diverse populations of youth and stakeholders.
PHYSICAL REQUIREMENTS
This is largely a sedentary position that requires speaking, hearing, seeing, and lifting small objects up to 10 lbs. May require the ability to travel locally and/or regionally.
