The Case Manager provides critical mental health support and services to LGBTQIA2S+ youth who live in Our Client's independent transitional living program.
The Case Manager relies on educational background, training, and clinical skills to develop and track each client's individual service and treatment plan.
This role helps address each client's basic and critical needs such as medical and mental health services, identifying community resources, accessing benefits, financial literacy, and finding stable and permanent housing.
The Case Manager works closely with support staff at Our Client's round the clock facility to implement programming and assist in meeting their treatment plan goals.
This role reports directly to the Program Coordinator.Tasks include, but are not limited to:
- Development and implementation of treatment plans for each client, with the goal of achieving independent living
- Crisis intervention as needed and referrals for psychiatric assessment
- Coordination of basic and critical care including medical, mental health, traumarelated care, substance abuse and other related support services
- Assist clients in their plan for independent living, including obtaining benefits, entitlements and other services
- Provide oncall support on a rotating schedule with other members of leadership
- Respond to all emergencies
- Weekly in person case management meetings with clients
- Develop and facilitate a life skills training program for clients
- Engage and maintain community partnerships to provide next level service for clients
- Regularly assess and adjust services provided to clients, collaborating with other staff and community partners
- Maintain accurate and timely documentation and reporting of services
- Preserve the highest level of confidentiality and integrity in the handling of all client information
Desired Skills and Qualifications
- Master's Degree in Social Work (MSW) Required
- Minimum of two years of experience in related fields of social services, human services or case management
- Experience working in a transitional housing program or a similar program
- Working knowledge of documentation for case management and record keeping
- Working knowledge of HMIS or a comparable system
- A professional knowledge of IPV, domestic violence or sexual assault and its impact on mental health
- A strong understanding of the intersection of identity
- Experience working with LGBTQIA2S+ youth
- Sensitivity to the ongoing needs and challenges LGBTQIA2S+ experience trauma, high risk for homelessness, abuse
- Understanding of substance abuse/recovery, medical and mental health treatment and family systems
- Working knowledge of local community providers, resources and services for LGBTQIA2S+ youth
- Selfstarter with the ability to multitask and identify priorities
- Strong organization and time management skills
- Exhibits excellent interpersonal and written communication
- Ability to remain calm, patient and understanding, even in times of crisis or escalation
Pay:
From $56,000 to $58,000 per year
