Arlington & Alexandria City PSH Case Manager


Reports to:
Director of PSH for Arlington County and Alexandria City


Supervises:
N/A


Summary of Position:

Some of the essential responsibilities of the this position are, but are not limited to:

  • Participating in team meetings with other Housing Case Managers to coordinate care;
  • Maintaining all pertinent program paperwork;
  • Coordinating moveouts, acting as the liaison between parties and guests when needed;
  • Providing crisis intervention and counseling as needed;
  • Meets with the guest to review housing plan. Regularly assesses, supports, and facilitates guest progress towards securing housing;
  • Completes rapid rehousing and prevention assistance paperwork for guests with an identified Housing Plan. Maintains records in compliance with state/federal guidelines;
  • Collaborates with shelter case management staff and other internal agency staff (Housing Locator, Community Case Manager, etc.) to provide financial support to the guest through ESG funds. Provides additional services for the guest through housing search support;
  • Communicates and interacts cooperatively with others, using a team approach in all situations;
  • Transporting guests when necessary in agency vehicles, including the agency's 15 passenger van;
  • Attending and participating in staff meetings, staff development and professional training to increase knowledge and enhance skills;
  • Completing case notes and documents all interactions and incidents in guest files;
  • Comprehending and adhering to ethical standards and confidentiality laws;
  • Participating in program case staff meetings;
  • Maintaining and updating guest files to meet contract and audit standards;
  • Modeling a caring, concerned, and respectful attitude toward guests;
  • Maintaining adherence to guest guidelines and upholds agency procedures;
  • Familiarity and complying with policies and procedures of New Hope Housing;
  • Role modeling positive behavior for all agency staff participate in agencywide events, and takes on the role of leadership as appropriate;
  • Performing other duties as assigned.

Preferred Qualifications:


  • Bachelor's Degree in social work or related field strongly preferred;
  • Two years' experience in a human service setting;
  • Spanish bilingual strongly preferred;
  • Understanding and commitment to "housing first and rapid rehousing";
  • Counseling and crisis intervention skills;
  • Ability to communicate effectively both orally and in writing;
  • Commitment to the mission, values, and philosophy of New Hope Housing;
  • Ability to successfully pass a background check and possess a valid driver's license with a good driving record to be approved by our insurance carrier;
  • Extensive knowledge of social, economic, health, and behavioral problems; ability to identify social problems and needs to assess residents' ability to utilize available services;
  • Ability to establish and maintain effective working relationships and advocate for service population and liaison with community resources;
  • Completion of collegelevel courses in sociology, psychology, or a related human service field; or any equivalent combination of experience and training which provides the required knowledge, skills, and abilities;
  • Demonstration of commitment to the specific agency mission values and philosophy;
  • Proficient use of computers to include Microsoft Office and other database software used to track service delivery.
