    Join the newly established Clinical Care Coordination and Discharge Planning team at UPMC, focused on guiding patients through their treatment journey with personalized care

RNs and social workers collaborate closely in discharge planning roles, acting as the central contact point throughout a patient's care journey alongside a Physician or APP.

    Key Responsibilities:

    • Adhere to system-wide competencies and behaviors
    • Act as an advocate for patients and their caregivers, ensuring access to necessary services while safeguarding their health, safety, and rights
    • Align practice with the organization's mission, vision, and values
    • Stay up-to-date with clinical knowledge and regulatory requirements
    • Collaborate with the care team to develop and execute individualized care plans
    • Utilize innovation and technology to enhance collaboration and patient outcomes
    • Facilitate efficient and safe discharge processes through team collaboration
    • Utilize area resources effectively to support post-hospital care arrangements
    • Identify and address various patient needs to ensure optimal outcomes
    • Communicate clearly with patients, caregivers, and the care team to create effective discharge plans
    • Lead process improvement initiatives to enhance care delivery

    Experience Needed in Discharge Planning:

    • Manage a patient's clinical care needs through transitions such as hospital to home or facility, and secure necessary resources for outpatient care
    • Coordinate services like insurance authorizations, transportation, and specialized care arrangements

    Nurse Track Requirements:

    • BSN qualification required

    Non-Nurse Track Requirements:

    • MSW or equivalent master's degree in a field supporting overall well-being required

    Licenses, Certifications, and Clearances:

    Nurse Track:

    • RN License Required
    • CCM/ACM or other relevant certification mandatory

    Non-Nurse Track:

    • LSW, LCSW or equivalent license required
    • CCM/ACM or other relevant certification mandatory

    Other Requirements:

    • Basic Life Support (BLS) OR Cardiopulmonary Resuscitation (CPR)
    • Act 33 clearance
    • Act 34 clearance
    • Act 73 FBI Clearance
