    GENERAL SUMMARY:
    Plans, organizes, develops and directs the social service programs to ensure that the medical related emotional and social needs of the residents are met and maintained on an individual basis


    PRINCIPLE DUTIES:
    1. Assesses and identifies medical-related social and emotional needs of the residents.

    Develops and implements social care plans with the involvement of the resident, his/her family, physician, and other nursing care staff.

    2.

    Consults with physicians and healthcare team members in dealing with psychosocial factors relating to residents' illnesses, treatments, and discharge plans.

    3. Provides discharge planning, including referrals to community and family social services. 4. Assists residents with financial and legal matters; provides or arranges for counseling. Assists residents with how to make decisions about their health. 5. Directly supervises staff. Carries out supervisory responsibilities in accordance with the organization's policies and applicable laws.

    Responsibilities include interviewing, hiring and training employees; planning, assigning, and directing work; completing performance reviews; rewarding and disciplining employees; addressing complaints and resolving problems.

    6. Possesses knowledge of federal and state regulations, including Medicare/Medicaid, to arrange for needed services not covered by state regulations. 7. Assists with the resident admission process. 8. Orients and trains employees on social services policies and procedures to ensure understanding and support. 9. Monitors supply and equipment needs and budget. 10. Implements policies and procedures to ensure compliance with federal and state regulations. 11. Develops and maintains community liaison activities.


    QUALIFICATIONS:

    Bachelor's Degree in Social Work from four-year college or university Minimum of two years related supervisor experience and/or training ACSW registration.
