    As a Senior Care Manager, you are responsible to provide a holistic, client-centered approach to caring for clients. This may include older adults, younger adults with disabilities, and individuals focused on improving quality of life. The Senior Care Manager provides assessments and continuous reassessment and manages comprehensive aspects of care


    Essential Duties and Responsibilities:

    • Responsible for Client Assessments and reassessments
    • Development of Care Plans and implementation of Care Plans
    • On a rotating basis responsible for initial client intake process
    • Uses initiative and independent judgment within established procedural guidelines
    • Works with formal and informal social support systems and families to ensure client's needs are met.
    • Coach clients and families through day-to-day and challenging life events.
    • Delegates tasks and provides guidance and support to the care management team.
    • Prepares complete and accurate case notes; writes correspondence, reports and other written materials; may prepare statistical reports and summaries. Develops and maintains working relationships with other department staff and partner agencies to coordinate services for families. Interact with other professionals on the client's behalf this may include, physicians, attorneys, fiduciaries, hospice personnel, and community personnel.
    • Prepares billing
    • Preparation of formal and informal presentations in the community as requested.
    • Available during business hours for client needs and on-call after hours when scheduled.
    • Create and maintain referral resources
    • Develops strategies to achieve business growth goals
    • Builds and maintains relationships with referral partners.
    • Ensures compliance with all federal, state, local, and department laws, regulations, and requirements.
    • Backup to colleagues as needed
    • Additional duties as assigned.

    Required Skills, Education and Certifications:

    • Bachelor's Degree in gerontology, health care, social work or related profession from an accredited university required; Masters Degree preferred
    • Minimum 5 years' experience in health care, elder care, social work or related industry preferred
    • Principles and practices of case management.
    • Codes, laws, and regulations related to the work.
    • Crisis intervention and family centered techniques.
    • Computer proficiency and ability to document timely and accurately notes in system related to client visits
    • Current driver's license and proof of insurance

    Physical Requirements:

    • Ability to travel approximately up to 30% of the time
    • Ability to lift and carry up to 15-20 pounds
    • Ability to sit, stand and walk for prolonged period of time throughout the work day
    • Ability to use standard office equipment such as personal computer, laptop, telephone, photocopy machine, etc.
