- Multifaceted social service agency serving elderly and persons with disabilities, seeks Consumer Directed Care (CDC) Case Manager. Responsibilities include assisting with implementation of CDC service delivery model by assessing and enrolling consumers. Providing ongoing support for Home Care consumers while meeting regulations and requirements. Provide assessment for CDC, support, and annual updates to SCO CDC consumers. Completing required CDC opening packet. Work closely with the Fiscal Intermediary to ensure that all forms are complete and accurate to initiate the services. Record keeping for Home Care CDC consumers and SCO CDC consumers.
- A hiring bonus will be awarded in 2 parts, half at 120 Days, and half at 1 year. The bonus for this position is $1,000.
