About Our Organization

The mission of the Latin American Health Alliance (LAHA) is to identify and address the significant health disparities that exist for Latinos living in the greater Worcester area.

LAHA advocates for the Latino Community; researches and responds to important social issues that affect the Worcester area Latino population.

LAHA implements initiatives and programs that address identified service gaps and continually evaluate the efficacy of our programs based on participant feedback.

LAHA is the only residential program providing culturally specific treatment for substance use disorder in the Worcester Community, we have been working for over 12 years to lessen health inequity in the Latinx community.

Over the past 12 years we have provided medically based residential treatment for Latino men suffering from substance use disorder at Hector Reyes House (HRH).

A significant gap was identified in safe, sober, affordable placement for the men leaving HRH so LAHA opened two transitional houses Casa, and Casa Mattie.

Hector Reyes House (HRH)

The Hector Reyes House is a new model of residential substance use treatment that provides services for 25 Latino men.

The program offers in-house medical treatment, along with intensive cognitive behavioral therapy and motivational interviewing.

HRH addresses nationally recognized health care disparities, alleviates a shortage of treatment options for Latinos, offers training and employment options designed to promote recovery, and reduce the odds of relapse.

Hector Reyes House is the only program of its kind in the region.


Position Summary

The Case Manager works closely with each of our patients, clinical team and medical staff to assist patients in achieving their goals, through individual sessions, group facilitation, treatment planning, family involvement, appointment scheduling, linking patients with community resources, coordinating discharge and after care planning.


ESSENTIAL DUTIES AND RESPONSIBILITIES
The essential functions include, but are not limited to the following:

  • Reduce the traditional power differential between Case Manager and patients
  • Be a key part of a collaborative teammanagement approach in delivering recovery and treatment programming
  • Work as a team with medical and clinical staff, and patients to determine recovery goals and goals for life outside of treatment
  • Work with patients to develop and implement goal
- oriented case plans based on patient's overall needs and hopes for the future

  • Meet patients individually on a weekly basis
  • Identify appropriate resources with patients, and advocate for services when needed
  • Facilitate groups
  • Provide leadership, advocacy, and mentorship to patients, staff, and the community
  • Provide crisis intervention services as needed
  • Ability to provide guidance on different pathways to recovery and being accepting of a variety of pathways to recovery, including Medication Assisted Recovery (MAR)
  • Meet with patients weekly for sessions/document session by end of shift
  • Assist Recovery Specialists with medications, transportation, groups, discharges, admissions, drug testing, household cleaning supplies, shopping, room searches, and noncompliance forms for patients who are not complying with program requirements
  • Accepting a team approach to household management
  • Any other tasks as assigned by Management

MINIMUM QUALIFICATIONS (KNOWLEDGE, SKILLS, AND ABILITIES)

  • Bilingual/Bicultural preferred
  • Strong written and verbal Spanish language skills
  • A personal interest in, and commitment to working with Latino men in Recovery
  • A flexible and positive attitude and belief in the process of recovery
  • Possess cultural sensitivity and adaptability to various personalities and background
  • Ability to work collaboratively with a multidisciplinary team
  • Must be committed to guiding patients in creating their own recovery plan, rather than simply dictating outcomes and expectations
  • Ability to recognize that onesizefitsall approaches to recovery and treatment models do not exist, but that individuals require unique treatment models to achieve their recovery goals
  • Must have an openminded approach to all forms of recovery including and especially MAR
  • Must have a basic understanding of the trauma

Pay:
$ $22.00 per hour

Expected hours: 40 per week
