Position Overview

We have a wonderful Full-Time Case Management opportunity with our Clinical Team at Evoke Wellness at Cohasset.


The role of Case Manager is responsible for creating a safe, comfortable and supportive environment for the patients participating in the program to receive the tools they need for early recovery.

He or she will provide individuals with support, understanding, empathy, education, treatment plans, groups, individualized care, and manage the case management aspect of the cases assigned under the direction of the Program Director and Clinical Supervisor.

Qualifications:
Position Responsibilities

  • Be the point person regarding clinical/case management concerns as it relates to patient care.
  • Completes all paperwork pertaining to patient, including but not limited to Biopsychosocial, Treatment plan, case management needs assessment, Assessments etc. in a concise timely manner.
  • Assures Compliance with expectations of attendance at trainings. (Including mandatory Relias Trainings)
  • Attends all clinical meetings
  • Enforces program rules and expectations and reports infractions to Program Director or Director of Clinical Services.
  • Monitors patient conformance with program schedule, reports problems and informs supervisor.
  • Develop and run groups as assigned based on relapse prevention, recovery, coping skills, independent living, process groups and other topics that will help those in recovery gain the skills and knowledge for a life in recovery.
  • Maintains the confidentiality of all patient and facility activities.
  • Complies with Policies and Procedures.
  • Accepts and utilizes supervision
  • Completes all tasks with concern for work quality and within the standards of compliance set by state, joint commissions, insurance, and federal guidelines.
Competencies

  • Demonstrates Proficiency in Communication, observation & Written skills.
  • Knowledge of Drug-Free Workplace Policies.
  • Knowledge of Workplace Violence.
  • Knowledge of Corporate Integrity & Ethics Knowledge of addictions.
  • Ability to interact with a multidisciplinary team to assist in facets of the recovery program relating to physical health.
  • Knowledge of the 12Step Recovery Program. Knowledge and competency in problem solving, stress management, ethics and team building.

Knowledge of alcohol and other drugs that includes:

  • Alcohol and addictive drugs and their physical, emotional, intellectual, and spiritual impact on the individual. o Alcohol and addictive drugs and their impact on the family.
  • Ability to determine if a medical emergency exists and to take appropriate action, when necessary.
  • Knowledge of emergency procedures used in case of alcohol and/or drug overdose.
  • Knowledge of First Aid and CPR.
  • Knowledge of Federal, State and Municipal laws and/or regulations that regulate the treatment of alcoholism and drug addiction, including: 42 CFR Part 2 Confidentiality HIPPA, Patient Rights, Professional Code of Ethics, Discrimination, Flexibility to adapt to schedule changes and assumption of responsibilities not delineated in the job description which are related to work as a member of an addiction treatment team.
  • Knowledge of Microsoft Office Programs (i.e. Excel, Word, and Outlook, etc.) and Google Suite (i.e. Docs, Sheets, Slides, etc.)
Education & Work Experience

  • High-School/GED
  • Sixmonth or more years of experience in the Human Services field or a related field.
  • CADC or LA
