    Interim HealthCare, a leading provider of home care services, is looking for an RN Case Manager in the Galveston area to join their team.

    Responsibilities of the Home Health Registered Nurse (RN) Case Manager:

    • Supervise and manage patient care from admission to discharge.
    • Coordinate patient care with clinical staff and medical providers.
    • Conduct home health visits and assessments.
    • Monitor patient progress and symptoms.
    • Develop personalized care plans based on patient assessments.
    • Administer medication, check vital signs, and provide wound care.
    • Educate patients following physician guidelines.
    • Document care accurately in patient records.
    • Offer home care to patients recovering from illness, injury, or surgery.

    Requirements for the Home Health RN Case Manager:

    • Completion of nursing school and current RN license in Texas.
    • OASIS training required.
    • Valid Driver's License.
    • Willingness to travel within designated area.
    • Minimum 1 year of experience in home health nursing.
