    Roles and Responsibilities of the Home Health RN Case Manager

    • Oversee and coordinate patient care from admission to discharge.
    • Collaborate with fellow clinical staff and medical providers.
    • Conduct various home health visits and assessments.
    • Monitor patient progress and symptoms.
    • Create personalized care plans based on patient assessments.
    • Administer treatments and medication as needed.
    • Educate patients as per physician instructions.
    • Maintain accurate patient records.
    • Deliver post-illness or post-surgery home care.

    Qualifications for the Home Health RN Case Manager Role

    • Graduate from an accredited nursing school with an active RN license in Texas.
    • OASIS training.
    • Valid Driver's License.
    • Openness to travel within designated area.
    • Minimum of 1 year experience in home health nursing.
