    Responsibilities of the Home Health Registered Nurse (RN) Case Manager:

    • Oversee and manage the entire care episode of patients, from admission to discharge
    • Collaborate with other clinical staff and medical providers to ensure comprehensive patient care
    • Conduct various home health visits such as admissions, recertifications, and discharges
    • Monitor and evaluate patient progress, reactions, and symptoms
    • Develop personalized care plans based on patient assessments and needs
    • Perform essential tasks like taking vital signs, administering medication, and providing wound care
    • Educate patients based on physician recommendations
    • Maintain accurate patient records in compliance with regulatory guidelines
    • Offer home care services to patients recovering from illness, injury, surgery, or those under pharmaceutical treatment

    Requirements for the Home Health Registered Nurse (RN) Case Manager Role:

    • Graduation from an accredited nursing school and a valid RN license in Texas
    • Completion of OASIS training
    • Possession of a Driver's License
    • Willingness to travel within the designated work area
    • Minimum of 1 year of experience in home health nursing
