    Interim HealthCare, a leading provider of home care services, is in search of an RN Case Manager in the Galveston area to join their dedicated team.

    Responsibilities of the Home Health RN Case Manager:

    • Supervise and manage patient care from admission to discharge.
    • Collaborate with other clinical staff and medical providers to coordinate patient care.
    • Conduct home health visits for admissions, recertifications, and discharges.
    • Monitor and evaluate patient progress, reactions, and symptoms.
    • Assess patient needs and create customized care plans.
    • Perform essential medical tasks like checking vital signs, administering medications, and providing wound care.
    • Educate patients based on physician recommendations.
    • Maintain accurate patient records following agency and regulatory guidelines.
    • Offer post-illness, injury, or surgery home care, as well as support for patients on medication or with acute conditions.

    Requirements for the Home Health RN Case Manager:

    • Graduation from an accredited nursing school and a valid RN license in Texas.
    • OASIS training.
    • Valid Driver's License.
    • Flexibility to travel within the assigned territory.
    • Minimum of 1 year experience in home health nursing.
