- Conducting home health visits for admissions, resumptions, recertifications, and discharges.
- Monitoring and documenting patient progress, reactions, and symptoms.
- Developing personalized care plans based on patient assessments.
- Performing various medical tasks such as checking vital signs, administering medication, and providing wound care.
- Educating patients as per physician recommendations.
- Documenting patient care following agency and regulatory guidelines.
- Providing home care for patients post-illness, injury, surgery, acute health conditions, or during pharmaceutical treatment.
- Must have graduated from an accredited nursing school and hold a valid RN license in Texas.
- Completion of OASIS training.
- Possess a valid Driver's License.
