SUMMARY DESCRIPTION OF WORK

This position will work in the Council Bluffs, IA Community Support Program based out of the Peer Center. The Community Support Program provides case management and supportive services to adults with a severe and persistent mental illness in the Council Bluffs community. Community Support services address mental health symptoms and functional disabilities that can create barriers for community integration and stability. The primary function of the Community Support worker is to create relationships with the population to provide a balance between support of the mental illness and allow for maximum individual independence.

This position is also responsible for some leadership responsibilities over the Community Support Specialist team. Leadership responsibilities may include orientation and training of new staff and ongoing support and guidance over program staff, documentation review and compliance, and ongoing development of the Community Support Program.

Compensation range: between $21.79 and $25.98 per hour (based on total years of relevant experience)
Work Schedule: 37.5 hours per week

MINIMUM QUALIFICATIONS

Bachelor's Degree in Human Services Field
Three years' experience working with Adults Living with Mental Illness
Valid Driver's License and acceptable driving record.

PREFERRED QUALIFICATIONS

Experience in case management
Experience working with area homeless service providers
Proficiency with MS Word & Excel

Essential Duties and Responsibilities

*Maintain responsibilities of the Community Support Specialist position.
*Provide program leadership
*Assist Program Coordinator in program development
*Takes a lead in role of quality improvement changes and Community Support programing.
*Orient, train, and support Community Support staff as designated by the coordinator.
*Is able to work with minimal supervision and as a part of the team
Monitoring productivity and mentoring staff to ensure program standards are maintained.
Ensure required client and program information is collected and entered into prescribed formats and reported in a timely manner.
Performs other program related duties as assigned.
*Create, maintain and share as appropriate a dynamic self-care plan.
Strive to make connections between the agency and the larger community whenever possible in order to contribute to the agency's ongoing fundraising and friend-raising efforts.
Essential functions of this job are to be performed on company physical work site or designated workspace.
