- Shift: Mon-Fri, 8AM-5PM (on call rotation)
- Unit: ACT
- Pay: Starting at $17.50 (Will increase to $18.76 once Case Manager certified)
- Rate increases based upon years of experience + certification
- Provides psychosocial rehabilitative services as described by the Medicaid manual
- Supports clients in following individualized care plans and identifying case management goals
- Completes documentation on services provided within Valley's standards
- Meets productivity expectations as assigned by supervisor
- Distributes medications as necessary
- Transports clients to appointments as necessary

- High School diploma or equivalent
- Case Manager certification must be obtained within 90 days | Valley will assist

- Driving positions require a current Driver's License, must be 21
- Previous behavioral health experience
- Bachelor's degree in social services, Psychology, or related field
