Valley Oaks is a group home for men specializing in maintaining mental health stability and sobriety. Case Managers will be responsible for passing medications, passing food at meal times, prompting independence and encouraging healthy habits.Case Managers provide psychosocial rehabilitative services as indicated by the client's care plan to support clients in meeting their individualized goals.
Case Managers are responsible for utilizing evidence-based practices, meeting fiscal responsibilities, and ensuring that regulatory compliance and organizational standards are met.
