- Creating and maintaining exceptional customer service and outreach activities to Santa Clara County's 52 nursing homes, as well as partnering health care organizations.
- Creating and implementing an outreach plan to Santa Clara County nursing homes and partnering health care organizations.
- Developing and maintaining regular face-to-face contact with referral sources
- Maintaining accurate documentation to track communication with referral sources, follow-through and results
- Creating exceptional relationships with potential and current referral sources
- Understanding CLC client population and clearly articulating program specifics with potential referral sources
- Effectively educating referral sources regarding the services offered by IOA and types of appropriate referrals
- Collaborating with referral sources and gathering pertinent information from potential referral sources to expedite client referral process
- Working collaboratively with Regional Director to attain census goals and identifying/solving problems or issues that impact referral goals
- Participating in required staff meetings
- Strong MS Office skills, particularly in Word and Excel
- Excellent interpersonal, customer service, oral and written communication skills
- Strong attention-to-detail orientation; capable of meeting deadlines
- Ability to use personal transportation throughout Santa Clara County
