    The Opportunity


    It's an incredible time to join us: we've hit an inflection point with fantastic patient engagement, overwhelming demand from physicians, and have recently unlocked substantial new revenue streams for our existing customers.

    However, we're still early — we want to fundamentally redesign the current paradigm of healthcare, and there are plenty of opportunities to contribute and make an outsized impact.

    Your Role in the Company


    As an outreach care coordinator, you'll play a critical part and have a strong impact on the total number of patients Gradia serves.

    You'll beresponsible for managing inbound and outbound calls to schedule appointments, and assisting embedded care teams with care gap outreach campaigns and pre-appointment planning.

    This patient-focused role will have a direct impact on our goal of successfully managing the comprehensive care of all of our patients.

    What you'll do

    Conduct outreach calls to patients with chronic conditions like IBS, GERD, and Fatty liver offering care program that will help them alleviate symptoms

    Handle phone calls and spearhead new patient enrollment

    Act as a liaison between the patient and the clinicians by providing high levels of customer service and resolving issues/concerns (if any)

    Handle phone calls and correspondence regarding new patient enrollment

    Requirements

    Customer service and/or sales work experience

    Experience in a customer service role requiring the use of enterprise software systems

    Demonstrated proficiency in computer software including spreadsheets, and calendaring

    Experience in a role requiring effective verbal, written, interpersonal communication, and collaboration skills

    Can work remotely under minimal supervision and take initiative over enrolling new patients
