JOB TITLE: TEAM LEADER
GENERAL STATEMENT OF RESPONSIBILITIES
The Team Leader will be responsible for the day-to-day supervision of the group home, day support, or in-home support program. The Team Leader will ensure that programs are properly staffed and billing information, along with other required documents, is submitted accurately and promptly. The Team Leader is also responsible for direct training of direct support professionals and routine supervision of services and must be available for calls 24 hours per day to handle needs or emergencies. The Team Leader makes on-going recommendations to the Coordinator regarding the changing needs of the individual, staffing patterns, and programs.
This position requires knowledge of confidentiality and security of records, Commonwealth Coordinated Care Plus (CCC Plus), Root Cause Analysis (RCA), Quality Improvement Plan (QIP), Department of Labor Regulations, Health Insurance Portability and Accounting Act (HIPAA), Occupational Safety and Health Administration (OSHA), (Department of Behavioral Health and Developmental Services (DBHDS), Office of Human Rights (OHR), and Medicaid Waiver and Home and Community-Based Setting (HCBS) regulations; individualized program development and evaluation techniques; personnel management and evaluation skills, supervisory skills; public relations, and oral and written communication skills; practical experience in behavioral support techniques, and individualized outcomes for special populations; individual rights, application and evaluation of assessment tools, medication management procedures, and experience in working with a variety of individuals with disabilities.
MAJOR DUTIES:
Provide relief for support staff as warranted
Directly supervise support staff
Provide required monthly supervision QA checks of support staff
Monitor upkeep of homes, apartments, independent living residences
Monitor documentation on MARs, presence of physician orders for prescriptions and over-the-counter medications and side effects information, sufficient medication status, disposition of outdated medications that are no longer used, proper medical supplies
Monitor individual's annual physicals, TB test, PAP smears, Prostate checks, and mammogram appointment
Ensure that appointments are set and met for all health issues that may arise
Conduct weekly meetings with support professionals (can be virtual or via phone)
Complete and monitor support professionals work schedules and ensure appropriate staffing
Review Individualized Service Plans with support professionals
Review documentation (Service docs, Support Logs, Learning Logs, Weekly review sheets, Behavioral Charts, Seizure Charts, MARs, etc.)
Review binders to ensure all required documents are present
Makes on-going recommendations to the Coordinator regarding individuals, programs, staffing patterns, and physical plant needs
Assist with adherence to Medicaid and Department of Labor regulations and standards
Assign and monitor activity schedules ensuring that all are submitted the by deadline
Ensure licensure standards are met and maintained
Maintain current, accurate, and appropriate individual records and other relevant documentation involving individuals and/or programming by reviewing QA information provided by the manager
Provide training in programming and behavioral assistance for individuals
Assist with providing training to support staff from ID Staff Orientation Workbook and Human Rights Regulations
Assist with providing consultation for behavior management techniques to staff and families
Train support staff on implementation of assessment devices
Assist with providing orientation for all new individuals and their families/care givers/representatives
Provide orientation for all new support professionals and staff training
Ensure and monitor individual transportation needs
Ensure adherence to first aid, CPR training, and behavioral management training
Assist with conducting support professional performance evaluations
Monitor health and safety management standards
Ensure adherence to all CIRCLE OF FRIENDS, LLC services policies and procedures
Be knowledgeable of current MR and MH services by attending workshops/seminars/conferences
Monitor communication between CIRCLE OF FRIENDS, LLC staff and families/care givers/representatives and other pertinent persons involved in the well-being of the individual
Ensure confidentiality and security of individual's records
Ensure that the Standards of Conduct for the company are met
Other responsibilities as assigned by the President, and/or designee
Attend and satisfactorily complete all required training in areas such as first aid, CPR, seizure control, medication management, behavioral supports, Medicaid Waiver information, etc.
QUALIFICATIONS:
BS in Special Education or related human services field, and a minimum of one (1) year of work experience with ID and ID/MH individuals preferred, or a combination of equivalent experience and training
Valid operator's driving license
Vehicle with current insurance
Good Criminal Records Report
