    Housing Case Manager

    Responsibilities of the Housing Case Manager is to provide services to adults who have a diagnosis mental illness, behavioral health issues, substance abuse, HIV/AIDs, and chronic homelessness. Specific duties included:

    • Case management services including benefit entitlements and case management.
    • Securing safe and affordable housing along with completing the necessary financial paperwork for a rental stipend.
    • Teaching the necessary skills to maintain stable housing.
    • Developing linkages to community services; and
    • Develop support plans for clients to utilize community resources when they need services or support.

    Requirements

    Skills/Qualifications for the Housing Case Manager include:

    • College degree in human services and/or equivalent experience in the human service field especially in the populations diagnosed with mentally illness, behavioral health issues, substance abuse, and chronic homelessness.
    • 1 year case management experience.
    • Valid and insurable driver's license.
    • Both the ability to work independently and as a team member.
    • Excellent communication skills both verbal and written; and
    • The ability to be organized and have good time management skills.

    Benefits

    Salary and Schedule of the Housing Case Manager:

    • 40 hours per week. Monday-Friday.
    • DAY TIME HOURS AND NO ON-CALL.
    • $21.87/hour. $45,489.60/ annual equivalent.
    • Overtime is available in our group home setting, $32.81/her hour for overtime pay.
    • Excellent benefits, staff training, generous paid time off, and 30-minute paid lunch.

    • Supported Housing Case Manager will have an average caseload of 25 clients.
