The
Case Manager ensures clients receive a coordinated range of client-centered services, linking clients with services following the Social Determinants of Health (SDoH) and the Social Determinants of Mental Health (SDoMH).
This position is responsible for completing client success plans and periodic follow up to address the needs identified in the plan; and for entering all client data, units of service, case notes, incident reports, and engagement activities into the database in a timely manner.
The Case Manager reports to the Director of Operations and Business Development.The salary is $24.72/Hr and Bi-lingual Spanish Required.
KEY ESSENTIAL FUNCTIONS:

Bilingual Spanish required:
- Assist and encourage clients to access the social services needed, including, but not limited to, medical care, financial wellness, housing, and access to available community resources.
- Conduct initial comprehensive assessment of clients' needs and support systems and develop a comprehensive, individualized service plan and coordination of the services required to address the identified needs.
- Maintain assigned clients record files and be responsible for files' completeness.
- Enter units of service and client data into appropriate electronic record and or reporting system on a timely basis.
- Monitor to assess the efficacy of the plan and periodic reevaluation and revision of the plan as necessary.
- Meet weekly with other program leaders to plan, review, and communicate client needs, program status, and needs.
- Connect client with services outside of the organization.
- Comply with program requirements, regulations, and processes in relation to sources of funding and organizational requirements.
REQUIREMENTS:
- Bachelor's degree in education, social work, sociology, or related field required.
- Spanish required. Must be fluent speaking, writing and reading both English and Spanish.**:
- Minimum of two years of work experience in social services, case management, and related field.
- Experience working in lowincome communities, schools, and/or communitybased organization (nonprofit).
- Clear and significant experience connecting clients with needed services.
- Attention to cultural competence in all manner of family and community engagement.
- Proficient in Microsoft Office and other online tools.
- Ability to thrive within a fastpaced, resultsdriven environment and work on multiple projects simultaneously and effectively.
