Eligibility Requirements
Graduated from an accredited nursing school
Holds a valid Registered Nurse (RN) license in the state of New York
A Master's Degree in Nursing is preferred
Minimum of two years of experience in a skilled nursing facility (SNF) setting
Proficient in the Minimum Data Set (MDS) and Care Planning process
Demonstrates strong leadership, interpersonal, and written communication skills
Committed to continuous professional development and growth
Key Responsibilities
Commence each shift by reviewing the daily log book
Conduct on-site assessments of assigned nursing units to evaluate the condition of each resident listed in the 24-hour report
Provide oversight to ensure the accurate and timely completion of care plans
Monitor the compliance of CHHC staff with the regulations set by the New York State Department of Health
Lead resident-centered care plan meetings, involving families, residents, and staff in the care planning process
Ensure proper maintenance of medical records following established policies and procedures
Foster an environment that actively engages residents, patients, families, and staff in tailored care and treatment plans
Officially declare resident deaths in the absence of a physician
