Position Summary:


Bedrock Healthcare at Home (BHAH) is looking for individuals to join their team of Wellness Ambassadors to drive the referral relationship between Bedrock and its partnered centers.

BHAH is a community based healthcare model that works with its partners in the industry to optimize discharge planning and patient outcomes through collaboration and continuity of care.

This individual will be stationed in a single facility (Hospital, Skilled Nursing Facility, Senior Living Community) with the sole purpose of maximizing conversions to Bedrock Ecosystem service lines through strategic and detailed collaboration with the facility team.


This individual will need to be a student of the industry with a mindset for providing great hospitality to its accounts.

Must develop an exceptional working knowledge of the services lines Bedrock offers including but not limited to Primary Care Physician Services, Medicare Part B therapy services, Continuous Care Management Services, and Private Duty Home Care.


This individual must be an excellent relationship builder and have a magnetic personality that allows for easy and smooth collaboration.

Needs be outgoing, compassionate, level-headed, and have a great Emotional Intelligence to ensure the dynamics and challenges of a high volume hospital/SNF are addressed.

Operates as a white glove team member of given partnered center.

Works with staff to ensure continuity of care by instilling strategic processes and procedures in place to automate discharges to the Bedrock Ecosystem service lines.

Understands that lowering readmission rates is vital to a successful partnership and by doing so the partnered facility can ensure quality outcomes measures for all patients under the Bedrock umbrella.


Essential Functions:


  • Works with facility team on a weekly basis to understand patient needs upon admission to hospital/skilled nursing and proactively sets up a discharge plan
  • Facilitates conversions to Continuous Care Management/Patient Care Therapy Group/Patient Care Medical Group/Bedrock at Home
  • Holds recurring Monthly partnership meeting with department heads to review wins/constructive action items
  • Weekly touchpoints with Bedrock onsite physician team to review wins/obstacles of week to week care and assists in addressing these items with facility staff (if applicable)
  • Provides referrals to partnered center
  • Reaches out to all leads in a timely fashion
  • Joins social work/case management team in weekly patient care conferences
  • Weekly meeting with admissions team to review each admission and the dynamics of the patient upon entering hospital/rehab
  • Weekly meeting with social work team to review upcoming discharges and ensures a plan is in place in a proactive fashion
  • Works with Bedrock Continuous Care Management team to develop outcomes report to be presented to facility leadership on a quarterly basis
  • Provides monthly report on referral conversions/wins/challenges/plan for subsequent month to Bedrock leadership
  • Resolves customer feedback by investigating opportunities for development, developing solutions, preparing reports, and collaborates on resolutions with program management
  • Excellent presentation, negotiation and relationshipbuilding skills

Non Essential Functions:


  • Attends all required training, inservices, and staff meetings.
  • Strives to maintain a safe working environment through the prevention of accidents, the preservation of equipment, the achievement of safe working practices, by practicing proper infection control measures, and understanding the use of Material Safety Data Sheets (MSDS).
  • Maintains a positive and professional demeanor toward residents, visitors, families, and coworkers.
  • Adheres to all policies and procedures of Bedrock Healthcare at Home
