- You'll support individuals in learning, maintaining, or improving skills through their participation in a variety of activities of everyday life, including in their homes and in the community to improve their level of independence
- You'll implement support strategies to achieve the individual's personalized goals
- You'll submit accurate and complete daily progress notes for each shift worked with the individuals
- Bachelor's Degree OR at least 2 years of comparable, community-based experience with adults with disabilities.
- Valid driver's license and existing insurance.
- Must be willing to drive individuals in personal cars throughout the community.
