    JOB SUMMARY

    Incumbent will serve as the Peer/Family Support Specialist for the Assisted Outpatient Treatment (AOT) services for Laura's Law AB 1421 eligible adults with serious mental illness in Santa Clara County and provide support to the Assertive Community Treatment (ACT) Team delivering ACT and AOT services.The Peer/Family Support Specialist utilizes his or her experiences as a recipient of substance abuse and/or mental health services to enhance recovery efforts of clients. He or she serves as a member of a multidisciplinary team and shares practical and personal experience, knowledge and insight related to illness management, life skills, and the lifelong process of recovery. The Peer/Family Support Specialist actively promotes greater understanding of clients' points of view and preferences. He or she maximizes client choice while also advocating on behalf of clients when appropriate. The Peer/Family Support Specialist also fulfills duties related to supportive counseling, rehabilitation, and collateral interactions with natural supports.

    • Performs as an active member of the AOT care team to provide outreach and engagement to AB1421 Laura's Law consumers.
    • Provide peer counseling and support, drawing on own experiences as a client to build trust and commonality.
    • Lead or co-lead client groups focused on wellness and recovery.
    • Provide outreach to potential clients and existing clients to engage them in clinic services.
    • Serve as a mentor to clients to promote empowerment and hope.
    • Provide expertise and consultation to other staff members from a mental health/substance abuse client perspective to enhance empathy and professional effectiveness.
    • Assist in the planning and implementation of client activities to promote wellness, socialization, and recovery.
    • Build relationships within the community to enhance client resources.
    • Promote a team culture characterized by openness to clients' experiences and encouragement of self-determination throughout treatment planning process.
    • Possess skills necessary to provide effective peer services, and the ability to successfully work as a member of a treatment team. Utilizes logical problem-solving techniques both independently and in collaboration with workers in dealing with client needs. Create written documentation that is legible, clear, and concise.
    • Advocate for clients by educating them about grievance or complaint procedures.
    • Comply with all MHS and program rules, policies and procedures including HIPAA standards.

    QUALIFICATIONS

    • Self-acknowledged substance abuse/mental health client who has or is receiving services.
    • Completion of the Recovery Innovations Peer Employment Training Program preferred.
    • Bachelor's degree and 2 years of experience, or high school with 4-6 years of experience working with consumers who have severe and persistent mental illness and co-occurring substance use disorder is preferred.
    • Proficient user of Microsoft Office Suite and EHR software.
    • Ability to problem solve and react appropriately to emergency situations.
    • Ability to work independently and effectively collaborate with a team.
