    Position Summary:

    The DSP- Activity and Community Facilitator's primary role is to provide Adult Day participants with the individualized support necessary for community integration and achievement of goals. Using participant's PCISPs, the DSP-Activity and Community facilitator will plan and implement outings and activities that promote independence, help participants reach goals and facilitate community integration.

    Essential Functions:

    1. Provide personal care to adult day clients who may require assistance in activities of daily living such as feeding, and toileting.

    2. Daily implementation of community integration and engagement- which may require driving a company vehicle or personal vehicle.

    3. Review participant's goals from their PCISP and plan activities that meet these goals.

    4. Assist with ongoing research of community integration opportunities.

    5. Create, record, and distribute monthly newsletter, with activities and outing choices communicated during member council meetings, self-advocacy groups, and one-on-one sessions.

    6. Assist with maintaining supply inventory list.

    7. Purchase activity supplies as directed.

    8. Keep receipts from participant outings and record and maintain monthly expenses and submit to ADS Manager.

    9. Record and maintain accurate monthly mileage logs and submit them to ADS Manager.

    10. Facilitate small and large group activities for all adult day service participants.

    11. Facilitate one-on-one activities for adult day service participants, as needed.

    12. Assist in documentation of daily record keeping, including documentation of adult day participants activities and goals.

    13. Assist with coordination of special events.

    14. Maintain open communication with families/caregivers, co-workers, and manager.

    15. Participate in staff meetings, in-service training, and other relevant training.

    16. Assist with ambulation of participants as directed and support physically impaired persons with good body mechanics.

    17. Observe and report changes in participants' condition to the ADS Manager or ADS nurse.

    18. Provide input and support to ADS Manager.

    19. Perform other duties as assigned.

    20. Maintain confidentiality of all Easterseals Crossroads participants in accordance with HIPPA guidelines.

    21. Practice environmental safety and sanitation.

    22. Ensure compliance of program operations and documentation with organizational policies and procedures, Medicaid Waiver Guidelines, CARF standards and other funding sources as required.

    Education, Licenses, or Certifications:

    1. High school diploma or equivalency required.
    2. · Valid Driver's license and Proof of auto insurance required
    3. · Experience driving passenger van preferred
    4. · Experience working in a group setting with individuals with disabilities preferred.
    5. · Experience with activity planning/implementation preferred.

    Experience required:

    Experience serving individuals with disabilities and/or special needs.
