Services offered

I work with attorneys nationwide who want efficient, professional, and cost-effective assistance with their lawsuits involving medical records. I work as a medical detective analyzing your medical records for active medical lawsuits as a behind-the-scenes expert. I specialize in home health and skilled nursing facility regulations. With a network of a variety of specialties, I can assist you with expert witnessing and expertise in a wide array of your medical case client needs to make your job easier and within budget. I'm dually certified for Skilled Home Health medical records/clinician documentation compliance of care and review of billing records.

My past experience is in leadership and management of quality assurance for internal auditing for deviance from standards of care or non-compliance by clinicians. Special skills include certifications to review clinician documentation for adherence to standards of care, medical necessity, billing, and compliance. My passion is helping attorneys leverage to get the best for their medical clients, and my advocacy will be toward medically harmed patients. Patients who have been harmed by the medical community need support to get past the resulting hardships and difficulties. I lend my medically trained brain and nursing expertise to the attorneys to see things in the medical records that are not there or are incorrectly performed by clinicians. I leave the legal expertise up to the attorney. I can help ask for documents in discoveries that are missing or write interrogatories to hold higher entities accountable. I offer a variety of report deliverables including but not limited to chronologies, root cause analysis reports, merit case review, demonstrative evidence, and DME/IME observation. Thank you for your consideration.

Experience

I enjoy working in medical administration and have experience in healthcare in many responsibilities including critical analysis of medical records, rendering informed opinions related to medical services outcomes, evaluation for root cause, auditing for compliance, and billing reviews. I also prepare medical chronologies, discovery requests, and interrogatory questions. I also have experience as an RN Clinical Manager, Acting Director of Clinical Services, and building a clinical team.

I gained experience at Anew Home Health as a Quality Auditing-RN in the Lafayette and Indianapolis territories with nurse and therapy teams, auditing medical charts and diagnosis coding. My primary responsibilities were reviewing and managing patient medical records from admission to collaboration of care between interdisciplinarians of all clinicians, clinician documentation of visits, functional scores, auditing visit narratives for medical necessity, review of careplans orders (POCs), auditing utilization of visit frequencies, setting the schedule for the episode, and monitoring progress through to the end of episode according to Medicare Guidelines. I also tracked compliance on Medicare timeliness for resumptions of care, transfers, discharges, and recertifications. I performed clinical review for intake of new admissions. I provided leadership in clinical decision-making for what is best for the patient and caregiver. I lead weekly nurse quality auditing training meetings to educate our team and build morale, along with weekly IDG meetings.

I also have experience working on billing and compliance audits for medical necessity, coding, HIPPS scores, comorbidity, and case mix review. I ran internal audits for complete billing review, PEPPER audits, Longer length of stay audits, and other audit types. I presented findings to leadership within each branch and enforced action plans and training opportunities to improve Value Based Purchasing within the company. Our department alternated taking questions from employees concerning regulations and F2F issues, and I was expected to be an expert in my field to assist branch leadership in clinical decision making.

I rely on my past experience at IU Health, where I served patients in need of bedside care, operating room procedures, PACU recovery care, and antepartum nursing. This included needing to be versatile according to the patient’s and team’s need of the moment and prioritizing to meet deadlines.

I  would look forward to the opportunity to work with you and support you as an RN Legal Nurse.

Thank you for your consideration and have a wonderful day.

Sincerely,

Charis Dulin RN-BSN, HCS-D, HCS-O, LNC
