    Job Description Job Description North Suffolk is looking for a Senior Care Coordinator to join our team Be proud of what you do Our goal is to ensure that children with behavioral/emotional needs, and their families, become independent within their own communities.

    The Senior Care Coordinator is not only about coordination, it is also assessing and identifying the underlying needs of the youth through regular contact with the family in order to make a direct positive impact in their lives using the Wraparound Service Model and providing supervision support and team leadership to other Care Coordinators on our team.

    We support families to recognize small areas of progress that builds into major changes.

    For each family that you work with, you will not only be the key contact for the youths providers, you will also be creating an individualized roadmap that guides uniform treatment among the team.

    As the contact, you will collaborate with multi-disciplinary teams such as PCP, school, DCF, Outpatient therapists and other mental health and/or medical providers.


    What Youll Do:
    Provide team leadership as well as clinical and administrative supervision to assigned Care Coordinators.
    Support the Program Manager with onboarding and training of new Care Coordinators.

    Gather information for the completion of strengths based comprehensive home-based assessments and support with the identification and development of natural supports within the familys networkOrganize, facilitate and document regular Care Planning Team meetingsDevelop a youth and family centered Individual Care Plan (ICP) that supports the achievement of goals identified with the familys voice and choice in conjunction with the care providers, natural supports and other identified team members.

    Maintain regular contact with the youth, family and other providers in the youths life by phone, community meetings or home visits to ensure implementation of the ICP.Provide education, advocacy and support to youth and families navigating through any of the child/family-serving systemsIdentify and connect youth and families to additional supports and services in the community through referral and coordination of careDevelop a risk management/safety plan and provide as needed crisis support, as well as coordination with the Mobile Crisis Intervention team.

    Support the Program Manager with the processing of incoming CSA referrals and management of the CSA waitlistSupport the Program Manager with the implementation of Utilization ReviewProvide on-call support under the direction of the Program ManagerWhat You'll Need:Education: Master's degree in a relevant field required

    Independent licensure preferredSkills/Experience:
    Must have Drivers License and reliable automobile.
    Minimum three years experience working with families with Serious & Emotionally Disturbed (SED) children, adolescents and their families.
    One year supervisory experience.

    Ideal candidate must thrive in fast paced work environments; have strong attention to detail, and demonstrate strong organization and multitasking skills.
