    **Description**

    The Family Peer Specialist (FPS) will provide non-clinical, active, hands-on peer support to parents/caregivers of youth receiving Wraparound services to build on the strengths of the family and help them to achieve desired outcomes. FPS services provide the family with skills, knowledge and support to strengthen the family and increase parents ability to support the treatment goals of the recipient and, thereby, to enhance the ability to function better within the home, school and community and to progress with recovery and improve resiliency. These services are: person-centered, focused on recovery and resiliency, and self-directed where possible. Effective peer support should be friendly, helpful, accessible and flexible. Peer support may be delivered in individual or group settings at the agency, in family homes, schools or other community environments. The role of the Family Peer Specialist is to provide peer support but also to work collaboratively to support systems change by increasing the familys involvement, education and skills while simultaneously decreasing unintentional bias about parents.

    **Essential Duties and Responsibilities**

    Can articulate the mission, vision and values of Choices, the values of System of Care and Wraparound.

    Decide collaboratively with the Wraparound Facilitator details about the initial engagement meeting with the family.

    Encourage the family to share their own story including their culture, beliefs and situation.

    Consider the familys story to identify family strengths, possible team members and what parts of the Family Peer Specialists story would be helpful for the family to hear.

    Effectively sharing your story in a way which builds connection, confidence and hope for the family relevant to the familys culture, beliefs and situation.

    Complete documentation in accordance with, state, federal, and organization requirements.

    Assess the level of support that is needed by the parent/caregiver based on the familys support network.

    Explore the parents situation regarding the need for rest, relief, and safety and work collaboratively with the Wraparound Facilitator to develop support stabilization.

    Work collaboratively with the family and facilitator to determine potential team members, both formal and informal and decide who will invite them.

    Provide crisis support as needed and follow-up with the Wraparound Facilitator to determine the need for additional support or revisions to the Plan of Care.

    Maintain direct communications with the parent/caregiver to ensure that their needs are being met and to ensure that interventions are effective. Including visits to the family home.

    Practice implementation and rehearse crisis responses with the family as identified in the Plan of Care and the Crisis Plan.

    Provide education and skill building to parents and caregivers that teach them to seek understanding and strategies for effective parenting.

    Teach strategies and services that help parents to identify family strengths and promote resiliency, and to develop natural supports.

    Provide support and helps parents to link to other parents for mutual support and learn how best to advocate for culturally appropriate services.

    Conduct weekly face to face visits with the family to assess child safety and family functioning.

    Participate with the Wraparound Facilitator and the family in planning and developing a family team meeting agenda and Plan of Care.

    **Qualifications**

    Be at least 21 years of age.

    Have a high school diploma or its equivalent.

    Must have experiences with child serving systems (i.e. mental health, child welfare, probation and special education).

    It is optimal for the Family Peer Specialist to have experience participating in Wraparound services or serving as a member of a Child and Family Team.

    Demonstrates an ability to maintain a non-judgmental attitude towards families.

    Demonstrate leadership and advocacy skills.

    Have strong dedication to family-driven and family-focused services.

    Strong communication skills. Bi-lingual skills (Spanish) a plus.

    Enthusiastic about advancing system change within the community.

    Must possess a valid drivers license in state of residence and auto insurance.
