Service Coordinator - Bingham Farms Pay: $20 - $24 per hour Schedule: Mon - Fri 8:30am - 5pm
JOB SUMMARY:
The Service Coordinator reports to the Hospice Administrator and possesses knowledge, training and experience in the Hospice Intake, Scheduling, and Coordination of Care Process
QUALIFICATIONS:
High school graduate or equivalent (GED). Preferably one years experience in healthcare or Hospice.
KNOWLEDGE AND ABILITY:
Knowledge of Hospice philosophy and end of life care. Ability to provide knowledge and skills on the computer. Knowledge of Hospice policies and procedures. Ability to exercise independent judgment. Ability to work with individuals.
JOB DUTIES:
Verifies that all required patient information is present prior to starting care. Utilizes appropriate clinical forms for entering data related to visit scheduling. Knowledge of hospice billing procedures. Submits timely computer documentation of all patient clinical visits. Responsible for answering telephone and forwarding calls to appropriate personnel.
Promptly follows up with ordering equipment, communicating new Admissions to the Pharmacy, and Coordinating IDG members with regards to patient care concerns.
Submits required documentation for billing purposes in the established time frame. Assists in implementation of policies and procedures. Observes confidentiality and safeguards all patient related information. Accepts responsibility for regular attendance and punctuality. File documentation/paperwork in appropriate chart All other duties as assigned.
