- Supports a patient's optimal wellness state by making outbound phone calls to patients/families recommending and facilitating the interaction with resources.
- Work is managed and often guided by precedent and/or documented procedures/regulations/professional standards with some interpretation.
- Understands own work area professional concepts/standards, regulations, strategies, and operating standards.
- Makes decisions regarding own work approach/priorities and follows direction.
- Always provides CenterWell Home Health gold standard of care.
- Minimum required education: LPN licensed in the state appropriate with no disciplinary action
- Experience with clinically assessing, planning, and advocating for patient's health needs
- Ability to document calls and interaction by typing on a keyboard while speaking to a patient on the phone
- Ability to work independently under general instructions and with a team
- Clear and concise communication
- 1-2 years related work experience in Home Health preferred
- Homecare Homebase (HCHB) system experience preferred
